Residential Respite Coordinator - Care to Serve - Burnie

Our client Care to Serve is a specialist non-profit organisation that exists to provide preventative support to Tasmania’s 85,000 + family and friend carer and is the Tasmanian provider of the National Carer Gateway support services. Care to Serve put carers first, connecting them to practical advice and early intervention support services.

About the role:

As the Residential Respite Support Coordinator, you will facilitate carer access to residential respite beds. This will involve communication and coordination with residential care facilities to identify and manage bed inventory, interacting with ACAT to ensure relevant and timely information is provided about care recipients, arranging the bookings, and completing booking confirmations.

Reporting to the Team Leader, Carer Support, you will also provide key support to carers from the suite of Carer Gateway services and participate in the on-call roster arrangements. You will build rapport with carers, gaining an understanding of their needs, and work with them to identify a support plan that will be most beneficial for the carer.

Responsibilities:

  • Receive booking inquiries from carers and identify their requirements
  • Confirm ACAT assessment or refer to ACAT assessment if not completed at the time of inquiry
  • Identify availability of bed requirements and arrange booking
  • Liaise with residential aged care facilities to identify bed availability
  • Assess the urgency of the interaction and make relevant referrals if urgent
  • Educate the carer on the services and programs available
  • Register the carer on the CRM, including recipient details, within the privacy and consent policies compliance
  • Ensure adherence to the safety policies of the organisation
  • Identify, report and proactively manage risks
  • Ensure all aspects of service provision is captured in the integrated CRM environment

The role will have a requirement to be on-call on a rotational roster for a couple of times annually. This is a full-time position working Monday to Friday.

What you will bring:

  • Experience working with family carers or with people from the CALD or ATSI communities or people with a disability is desirable
  • Certificate III in Community Services or higher or equivalent or equivalent demonstrated experience
  • Demonstrated experience in applying a person-centred approach within a client-directed environment
  • Proven ability to communicate flexibly and effectively at all levels and get to the root cause of issues
  • Demonstrated experience in solving problems with initiative and speed within an acceptable range of risk
  • Demonstrated experience in assessing, planning, implementing and reviewing support plans
  • Proven experience working with integrated systems and ensuring high levels of accuracy
  • Proven ability to work productively within a team environment

Desirable / preferred, but not essential:

  • Membership of relevant professional association viewed favourably
  • Previous work in the community services sector is highly desirable.

  • Current valid driver’s license
  • Up to date COVID vaccinations (including boosters as required)
