We are looking for an experienced and passionate Case Manager to join our Brunswick West, Youth Residential Support Service. You will be responsible for providing proactive case management to clients within the Refuge and also outreach support to past residents who have transitioned into new accommodation. Your client-centred case management approach will encompass trauma-informed practice with a focus on empowerment, wellbeing, positive housing outcomes, engagement and community participation.
You will also assist Young people to access external funding such as Private Rental Brokerage or Housing Establishment Funds for tenancy expenses, tenancy settlement, or emergency accommodation. This service is designed to provide young people with intensive case management for a period of six weeks, though many clients may receive longer support due to the complexity of their situation.
Reporting to the Team Leader you will form part of a multi-disciplinary approach team and specialist youth homelessness workers within North-East Services as well as across the organisation.
