Huddle Community Services (HCS) is a small specialised team that supports people with diverse and complex needs who have an NDIS Plan. HCS is looking for an innovative and creative person to join the team and bring their skills in community engagement, outreach, navigating systems and using a solutions focused approach to work within complex situations.

HCS is recruiting for the role of Support Coordinator/Psychosocial Recovery Coach. A Support Coordinator and Recovery Coach at HCS works with clients who have NDIS Plans to build a holistic support network based on the individual’s specific needs, preferences and circumstances. This role works collaboratively with other services, ensures clients have choice and control in the supports they access, and supports clients to effectively utilise their NDIS funding to meet their needs. HCS has a strong focus on minimising the barriers to inclusion and engagement, and uses a strengths based approach to understand each client’s individual support needs.

Key Tasks & Responsibilities;

  • Set-Up and Coordinate services in participants' NDIS plans including the management of complex multidisciplinary teams.
  • Deliver high quality services in a timely manner including mandatory reporting to the NDIA QSC and meeting HCS guidelines.
  • Provide outreach, advice and consultation with participants, their families and other stakeholders in response to complex and challenging situations.
  • Develop action plans following consultation with participants, their families and other stakeholders to resolve challenges arising from complexity.
  • Provide support and advice to other support coordinators in relation to service delivery.
  • Liaise and collaborate with key stakeholders to establish mutual goals of coordinating service delivery for participants.
  • Facilitate and participate in meetings with the team including supervision and de-briefing support.
  • Prepare comprehensive reports in conjunction with relevant stakeholders.
  • Contribute to the maintenance of registers, reporting systems and client records ensuring the need to adhere to matters of confidentiality and diversity within a sensitive environment.
  • Maintain accurate and complete records of your work activities in accordance with legislative requirements and HCS's records, information security and privacy policies and requirements.

Qualifications & Experience Requirements;

  • Minimum Cert IV in Disability/Mental Health and/or Alcohol and Other Drugs or equivalent (Diploma of Community Services, Bachelor of Social Work/Psychology/Behavioural Science).
  • Relevant experience in working with people from diverse backgrounds and with varying needs; Homelessness, Mental Health, Alcohol and Other Drugs, Refugee and Asylum Seekers.
  • Valid Victorian Driver's license.
  • Successful completion of Working with Children Check and NDIS Worker Screening Check.
  • Strong written and verbal communication skills, including report writing.
