It was established to engage, support and empower young people to build on their strengths, increase their ability to participate in society, and contribute to more socially inclusive communities. ABOUT THE ROLE

Reporting directly to the Head of Social Cohesion, the primary responsibilities of the Case Manager (contract - 24 months) will include:

  • Delivering high-quality and culturally appropriate initial intake assessments, case management, and referrals for children and young people from multicultural backgrounds.
  • Completing in-depth psycho-social assessment to determine the person’s personal circumstances and the level of support and type of interventions required.
  • Assisting children, young people, and their families to access specialist support services including (but not limited to): mental health, drugs and alcohol, homelessness, crime prevention, and family violence.
  • Establishing and maintaining partnerships with Victoria Police, hospitals, local schools, and religious leaders, amongst others to proactively and strategically prevent risk factors for anti-social behaviours amongst children and young people.
  • Promoting the safety and wellbeing of children and young people by adhering to The Huddle’s Child Safety Code of Conduct.
  • Implementing case management meetings, participating in service coordination, and identifying pathways for children and young people including risk assessments and safety plans for individuals.

OUR IDEAL CANDIDATE

The ideal candidate will possess the following qualifications, skills, and attributes:

  • Demonstrated understanding of issues and challenges experienced by Somalian Australian community children and young people in North Melbourne and Wyndham.
  • Proven ability to reach and support disengaged children and young people from culturally diverse backgrounds.
  • Experience working within a case management framework and understanding of the ‘system’ to support at-risk and vulnerable children and young people is highly regarded.
  • Demonstrated experience in planning, delivering, and evaluating interventions and development initiatives to support outcomes for at-risk and vulnerable children and young people from culturally diverse backgrounds.
  • Experience in communicating with people who may have limited proficiency in English. Excellent communication skills (oral and written) in English and Somali, Arabic, or Tigrinya are highly desirable.

Our ideal candidate is capable of working at multiple locations and a wide array of hours with a varying schedule, including after hours and weekends

The successful candidate will also be required to hold a valid Victorian Working with Children Check and undertake a Police Check.
