In recognition of the benefits of social connection, Access HC are growing a social prescription program to connect socially isolated individuals with community. We are looking for patient, empathetic volunteers, to build rapport with socially isolated people to support identification of suitable activities and increase engagement in community.
Access Health and Community’s 'Access to Community' program aims to engage 'Home and Community Care Program for Younger People' eligible community members who may be experiencing loneliness or social isolation and connect them with community programs/activities based on their identified interests/needs across Manningham, Yarra and Boroondara local government areas.
Volunteer Community Connectors help to support clients to access activities and follow through on their goals. This involves meeting with the client to talk about what matters to them, then researching and reaching out to community organisations to identify suitable community programs and supporting the client to choose the most suitable option. The role involves offering practical and emotional support to help with confidence, for example helping to find the right bus route or making the journey with the client due to psychological barriers or poor mobility skills. The volunteer’s role within the Access to Community program is varied and dynamic.
Community Connectors will need to be strengths based, client centred, patient and empathetic.
Access HC values it's volunteer workforce and provides training opportunities and ongoing supervision.
The program will is being piloted in Boroondara, soon to expand to Cities of Yarra and Manningham. This role would suit Psychology/OT/Social Work/Cert IV Mental Health students seeking experience with vulnerable and isolated people.
Proficiency in languages other than English highly regarded.
