The opportunity with Interchange:

The Alternative Home Living Coordinator will be responsible for the management of our Alternative Home Living programme, which ensures customers can live safe and well in their home. This role, which is end to end, is responsible for the promotion of the service, recruitment and onboarding of participants and customers, as well as monitoring and supporting to ensure the service offered by the arrangement, meets all aims and objectives, including customer goals.

The main goal of the program is to promote the service, identify active participants and assess to understand what prospective living arrangements could make a positive impact in their life. Alternative Home Living programme could include host or companion arrangements, home share, respite hosts or appropriate neighbourhood volunteer arrangements. These natural arrangements will allow people with disability to experience an enhanced sense of connection and belonging in their community.

To join the Interchange Team, you need to have:

Essential qualifications, knowledge or experience

  • Possession of relevant tertiary qualifications in disability services, community services or social work, or related professional experience and or proven experience in the field or demonstrated experience in a relatable or similar role
  • Knowledge in coordination of supports / case management to assist people with a disability in their community or in their home.
  • A demonstrated commitment to the values, vision and mission of Interchange.
  • A demonstrated understanding of and experience in the development and delivery of contemporary, evidenced based disability services.
  • Demonstrated ability to foster and maintain positive working relationships with customers, stakeholders and work effectively with a diverse range of individuals and communities.
  • Highly developed interpersonal skills to positively build drive engagement, motivate staff and influence culture.
  • Excellent verbal and written communication skills including report writing with a high level of attention to detail, accuracy, and confidentiality.
  • Excellent time management skills and demonstrated ability to effectively manage multiple projects, effectively set priorities.
  • Demonstrated commitment to continuous improvement.
  • Demonstrated proficiency in a wide range of IT applications.

  • Contemporary and positive attitude toward people with disabilities
  • Satisfactory Police Clearance Certificate (issued within previous 3 months)
  • Current WA Driver’s license
