We are looking for two Support Coordinators that share a passion to help people with disabilities live a good life. These are part time opportunities working a minimum of 19 hours per week for a fixed term of 12 months with flexibility to work from our Maddington Head Office and other Neighbourhood locations.
You will work in partnership with the people that we support to identify their goals and aspirations. You will develop those goals into a NDIS Support Plan that will provide access to a variety of support services that will allow the people we support to achieve their desired outcomes, including coordination, assessment and implementation of a range of supports within their funded plans.
We need someone that will be influenced by their knowledge of a person-centered approach to develop individualised strategies and outcomes for the people we support.
