We are looking for a committed and experienced Antenatal Social Worker to work as part of our innovative Child, Youth and Family Team. This team provides a range of services to children, young people, families and individuals who are often isolated and on low incomes.

This is a unique role which responds to vulnerable pregnant women with complex health needs in the City of Port Phillip.

The Antenatal Social Work position involves the provision of assertive outreach case management to pregnant women, advocating with other services in relation to the needs of marginalised pregnant women, and connecting women with relevant internal and external services e.g. maternal and child health and housing services.

Ideally, you will possess:

  • A professional qualification in Social Work
  • Experience in the provision of assertive outreach programs and services
  • Demonstrated ability to work independently as a sole outreach worker with proven ability to work with marginalised and vulnerable clients
  • Excellent knowledge of the social model of health
  • A clear understanding of antenatal care options and parenting supports
