An exciting opportunity exists for an experienced case manager to work with the H2H Team. This is a fixed term position of twelve months.

Join a team that has been providing assertive outreach case management support to people who have experienced long term significant homelessness and who are now housed with the goal to assist with sustaining their tenancies.

Be part of an opportunity to facilitate stability in the lives of people who have experienced considerable disadvantage.

This role will suit candidates who have experience supporting clients with complex needs, possess Counselling skills
or are competent in Duel Diagnosis.

A current Victorian Drivers Licence is essential.

Ideally, you will possess:

  • A professional qualification in either Social Work, Psychology, Counselling, Community Services
  • Experience in Case Management in the Homelessness, Mental Health and/or AOD sectors
  • Demonstrated ability to provide client centred, trauma informed practice. Build rapport with complex client cohort
  • Excellent knowledge of Homelessness, Mental Health Support Systems and Social Housing.
  • A clear understanding of Sustaining Tenancies framework, barriers to sustaining tenancies for H2H clients
