An opportunity exists to promote the health and wellbeing of older tenants living in public housing by improving their access to a range of support services and strengthening their sense of safety and connection with the communities in which they live. The role includes the provision of short term case management and community development.  

Ideally, you will possess:

• A professional qualification as an RN Division 1 Nurse / Allied Health Professional or Social Worker
• Experience within a community context
• Demonstrated ability to provide services to older people from diverse backgrounds with complex needs who are socially isolated and/or have challenging behaviours
• Excellent knowledge of the health, aged and disability service system and the public housing sector
• Skills in Community Development approaches
• Knowledge and skills in the use of clinical assessment systems, care planning and case management
