An opportunity to promote the health and wellbeing of older, isolated and vulnerable residents at 27 king street , by providing access to support and services . The role includes facilitating community development activities in consultation with the residents to provide a flexible and responsive service to meet their needs.

Ideally, you will possess:

• A professional qualification as an Allied Health Professional, RN Div 1 or Social Worker
• Experience in community health settings
• Demonstrated ability to provide services to people form diverse backgrounds with complex needs
• Excellent knowledge of health , aged and disability services
• A clear understanding of the social model of health and community development principles
