An exciting opportunity exists for an enthusiastic and experienced Team Leader to join our Star Health Community Wellbeing Support Service for Older Adults. This is a newly funded program by South Eastern Melbourne PHN will provide highly flexible, timely and accessible support to aged consumers in the community who have had their access to community based support services interrupted by COVID. Care coordination, practical support liaison and outreach support will be among the suite of flexible services to assist and empower consumers to achieve greater health outcomes.

The Team Leader will have a senior role in the multidisciplinary Community Wellbeing Program providing clinical and non-clinical care coordination to consumers and supervision to team members.

Ideally, you will possess:

• A undergraduate degree qualification in Social Work
• Experience in delivering evidence-based therapeutic interventions to older persons and supervising staff
• Demonstrated ability to create person-centred outcomes in the context of psychological service delivery
• Excellent knowledge of best practice and effective treatments when working with older adults and stakeholders
• A clear understanding of and commitment to the Social Model of Health
