We have amazing clients across the Ryde/Parramatta & Bankstown region who are funded under NDIS, Home Care Packages, privately and brokered through other schemes.

We are currently recruiting a Package Manager to join our passionate, supportive office team in Newington.

Who you are

Due to continued growth of services, we are looking for an experienced and motivated NDIS/Home Care Package Manager to join our team. This role is instrumental in providing support and advice to the Home Care Packages & NDIS clients to assist them to continue living at home. Your responsibilities will include day to day management of a client case load, conducting assessments, care planning and provision of consumer directed services to complex care clients within budget guidelines. You will build professional relationships internally and externally and look for creative care solutions to achieve the best outcomes for clients. This role involves travelling to meet clients in the community as well as working in a busy office environment.

  • Qualifications in Community Services, Human Services, Nursing or equivalent.
  • Previous experience in Aged Care or Community Services.
  • Ability to work flexibly to respond to changing customers’ needs.
  • Ability to coordinate services according to relevant program guidelines.
  • Experience of working in a multi-disciplinary team.
  • Ability to develop and maintain links and supportive networks with specialist services.
  • Experience maintaining computerised customer information systems.
  • High level administrative and organisational skills.
  • High level of written and verbal communication skills.
  • Demonstrated ability to contribute positively to the team.
  • Current and clear Criminal Record Check.
  • Current Working with Children Check (where required in accordance with state based legislation)
  • Current Drivers Licence.
  • NDIS Worker Screening Check
  • COVID vaccinated + boosters (essential)
  • Previous experience or transferrable skills are ideal, eg previous coordination roles in Aged/Disability sector

What’s the job?

The Just Better Care Package Managers perform an integral role in assisting in individualised services being delivered to customers.

Responsibilities include:

  • Assist in customer service schedules including service planning, confirmation and short notice placements or cancellations.
  • Liaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal and other regulatory bodies.
  • Ensure care plans are monitored and reviewed on a regular and ongoing basis and in response to changes in customer needs.
  • Ensure services are delivered are in line with the person centred and consumer directed principles.
  • Assist customers to access other alternative services where the services are no longer meeting their needs.
  • Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectives.
  • Monitor customers’ budgets in accordance with program guidelines.
  • Act on care need recommendations by Health Care Practitioners.
  • Identify and participate in personal professional development.
  • Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.
  • Communication with key stakeholders, internally and externally.
  • Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.
  • Any other duties as required to meet the ongoing needs of the organisation
