About the opportunity:

The Advanced Case Manager position sits within the Rough Sleepers Initiative program. It provides short-term and comprehensive housing focused support for people sleeping rough in the City of Melbourne. The worker will support up to six clients at a time, for up to three months (up to 46 clients during the project), with the aim of completing important tasks essential to achieving key goals which assist in moving the person along the pathways to obtaining both suitable housing and longer-term support.

Working in conjunction with the existing Service Coordination Project (SCP), this intervention will support clients on the By Name List and will receive referrals from the SCP weekly team meeting. The project is targeted at individuals who are entrenched in their rough sleeping, and for whom there are significant barriers impeding their pathway to housing, which can be navigated and overcome through the provision of this comprehensive support intervention. Key tasks may include: a deep housing needs assessment; referrals to, and connection with, psychiatric services; support with applications for Public Housing, Community Housing, NDIS, DSP and MACNI.

This is a max-term, part-time appointment until the end of June 2025, based in St Kilda at 45.6 hours a fortnight.

What you'll be doing:

  • Work with Service Coordination partners to identify and prioritise people experiencing chronic homelessness who require in-depth assessments that current service system efforts have been unable to secure.
  • Undertake in-person assessments of needs and requirements for this group of people to assist with prioritization and case coordination, including identification of the correct service/s to engage with the person, supporting the service/s to engage, the development of plans of engagement, and the identification of system gaps and required resources for escalation to project governance.
  • Make regular presentations to Service Coordination and to assist them to understand the needs and requirements of this group including service gaps and barriers.
  • High level advocacy and escalation of referrals of clients whose needs are not being met by the existing service system
  • Provide reports and briefings on activities and outcomes.
  • Support a culture of trust and inclusion and contribute to a program of continuous improvement by encouraging constructive feedback from stakeholders to inform program and service improvements.
  • Ensure the work complies with funding and service agreements, professional standards and relevant legislation, policies and guidelines.

What we're looking for (key selection criteria):

  • A relevant tertiary qualification (desirable).
  • Significant experience (more than 3 years) as a specialist practitioner.
  • Homelessness or related services (i.e. Nursing, mental and primary health) which intersect with homelessness.
  • Demonstrated understanding of the issues which those experiencing homelessness experience, and skills and aptitude in working for people who are marginalised and disenfranchised.
  • Excellent people skills, including a demonstrated capacity to work positively with a range of diverse stakeholders.
  • Very high level written and analytic skills.
  • Highly developed communication, facilitation and presentation capabilities and a desire for continuous quality improvement.
  • Superior organisational skills and experience in dealing with complexity, juggling priorities and working to deadlines coupled with a solid work ethic.
  • Experience using the SHIP/SRS Client Management System (desirable).
  • Demonstrated commitment to the values of Launch Housing and our mission to end homelessness.
