Why this role matters

The position is targeted to people experiencing homelessness and currently accommodated in hotel and motel accommodation during the COVID-19 pandemic. The target group will be people with complex histories of trauma and homelessness and with varied support needs. The role will be responsible for supporting clients who will transition from hotels to alternative accommodation including head leased properties in the private rental market. The support provision will deliver integrated and flexible housing and wellbeing focused case management support, to address immediate crisis support needs, through to longer term support to establish safe and affordable long-term housing.

What we're looking for

We are currently seeking a skilled & passionate individual, someone committed to working with people who are experiencing homelessness, or at risk of homelessness on their journey out of homelessness.

Key Responsibilities:

  • To deliver integrated and flexible housing and wellbeing focused case management support
  • Assist clients with the successful transition and establishment in head leased housing
  • Develop case plans with clients who experience multiple and complex needs, and implement strategies that enable clients to achieve their goals by building on their strengths and focusing on solutions.
  • Assist clients to access affordable longer-term housing options, both within and/or independently of Launch Housing's accommodation options
  • Initiate and maintain contact with vulnerable clients using flexible engagement strategies to achieve case plan goals, including assertive and persistent outreach.
  • Build relationships and work collaboratively with specialist services to provide a coordinated response to improve clients physical and mental health and wellbeing.
  • Ensure clients maximise all government and community income assistance and support.
  • Advocate on behalf of clients with other agencies to ensure access and delivery of services.
  • Maintain client files and records in accordance with organisational procedures on a computer case management software system.
  • Actively participate in organisational structures and meetings, including team meetings with a client focused, continuous improvement mindset.
  • Maintain key, relevant relationships and proactively build Launch Housing's brand and enhance client outcomes.
  • Facilitate close working relationships with key service providers
  • Ensure clients are referred to appropriate supports
  • Cooperate and comply with all risk, health & safety and compliance policies, frameworks and procedures
  • Comply with agency reporting requirements, organisational policies and procedures
  • Work within Launch Housing's ethical, privacy, OH&S and duty of care frameworks
