Using the Consumer Directed Care approach, as the Care Advisor you will work with older people to identify services to meet their needs, to support their physical, social and emotional wellbeing as they continue to live at home. Each Care Advisor works directly with older people who have been allocated a Commonwealth Home Care Package to determine what services and supports they require.

To be considered for this role you will require

  • A relevant qualification or willingness to obtain (e.g. Associate/Diploma of Case Management and/or Community Services /Nursing/ or related field) or relevant work experience
  • An understanding of Consumer Directed Care principles
  • An understanding and experience of client assessment and care planning for older people
  • Financial skills to effectively develop and monitor package expenditure
  • A current valid Victorian driver licence.
