Using the Consumer Directed Care approach, you will provide services to aged care consumers on Home Care Packages while their allocated Care Advisor is on leave. You will work with older people to identify services to meet their needs, and to support their physical, social and emotional wellbeing as they continue to live at home.

As a Leave Coverage Worker you will perform the work of a Care Advisor for consumers whose allocated Care Advisor is on leave to ensure continuity of service. The consumers who are supported receive a Commonwealth Home Care Package to assist them to remain living at home.

To be considered for this role you will require

  • A relevant qualification (e.g. Associate/Diploma of Case Management and/or Community Services /Nursing/ or related field) or relevant work experience
  • An understanding of Consumer Directed Care principles
  • An understanding and experience of client assessment and care planning for older people
  • Financial skills to effectively develop and monitor package expenditure
  • A current Victorian driver licence
