As a Linkages Care Coordinator, you will assist people who have fallen through the cracks or aren’t adequately linked in to existing services.

To be successful, you will require;

  • Tertiary level qualifications in the Aged Care industry
  • Extensive experience in an equivalent of similar role
  • Thorough understanding and use of the HACC-PYP, CHSP or HCP or NDIS programs in previous lines of work
  • Current valid Victorian driver licence
  • An Employee Working with Children Check (must be obtained prior to appointment)
