We are currently looking for caring and enthusiastic Support Workers to join our fast-growing Perth team!

About us

Lifeplan is a not-for-profit organisation that provides disability support services to people of all ages throughout Perth, Western Australia. Established in 1999, Lifeplan is an NDIS registered service provider that prides itself on working collaboratively with local co-ordinators, planners, guardians and accommodation providers. 

As a service provider, Lifeplan’s work is focused on always being alongside customers, supporting them to take charge of their own lives; and helping them to participate fully within their self-identified community. Those services are tailored to support customers and their families to choose the support they need, who will provide it, and how it will be delivered. Individualised customer service, and matching support workers to customers are central to the work that we do.

We also enjoy a laugh, and we embrace diversity, inclusion and creativity. It all adds to the fabric of Lifeplan.

The role 

No two days are the same but a ‘typical’ day may include accompanying our customers to:

  • the library, park, pool, shops or other community-based activities
  • attending medical appointments
  • a visit to see family or friends

Your day may also include:

  • dropping into head office to pick up a van or to touch base with your support team
  • writing up notes and completing your time-sheets in our rostering application (training provided)
  • attending training or workshops

Work days range from Monday – Sunday (no nights)

  • You will need to be flexible and open to learning new things
  • You will need to be adaptable and understanding as plans can change suddenly and you will need to be willing to ‘go with the flow’
  • You will ensure that you remain up to date with your competencies
  • You will have a person-centred approach
  • •very customer is different and we expect you to provide quality individualised support to help them meet their goals
  • You will have an understanding that you are a part of the Lifeplan team and will ensure that you respect and adhere to the organisations vision and values
  • You will need to have an understanding of NDIS and a commitment to its principles

Requirements

  • An unrestricted current driver's licence
  • A safe, reliable, registered vehicle with comprehensive insurance to provide customer transport
  • Current NDIS Worker Screening check or willingness to obtain one
  • A working smart phone
  • Australian passport or relevant Visa to demonstrate working rights in Australia
  • Excellent communication skills including written and verbal
  • Availability to attend training or meetings when required
  • Ability to problem solve, think creatively and find innovative solutions
  • The Government has now made it mandatory for all workers within our industry to be fully vaccinated against COVID-19. We therefore require all applicants to provide evidence of their COVID-19 vaccination as part of their application
