Lifestyle Solutions are seeking an experienced Support Coordinator to join our small but committed team in North Western Sydney as part of our national team of Support Coordinators. In this Support Coordinator role, you will advise customers of appropriate supports and services to strengthen their connection with their local community, build skills and overcome barriers with the overarching objective of assisting customers create independence and achieving their goals. You will also be communicating and developing relationships with customer families, guardians and external service providers to further assist with customer engagement and connectedness.
