About the Role

As a Case Manager within our aged care program, you will be providing support to clients who access our Home Care Packages in Ballina and surrounds. You will have the opportunity to build relationships with various stakeholders, staff and clients and ensure clients have up-to-date care plans and access to services and resources that provide them with holistic care.

This is a permanent full-time role with some travel required to Ballina and surrounds to meet with the clients and relevant stakeholders. However, flexibility is provided to work from home or an office of your choosing for the majority of your working week.

Key Responsibilities

  • Undertake comprehensive consumer assessments, care planning and review.
  • Develop strategies and/or broker services to address the client needs in consultation with families and communities.
  • Allocate home care workers and maintain changes.
  • Maintain regular communication with consumers and their families.
  • Liaise with community-based agencies and service providers to ensure an integrated and cohesive approach to service delivery.

 Skills & Experience

  • Degree or equivalent in human services, nursing or related field.
  • Previous Case Management experience and a passion for working within the Aged Care field.
  • Current Drivers Licence.
  • Demonstrated experience of working with older people, including people from culturally and linguistically diverse backgrounds, and understanding of issues facing people with chronic and complex needs.
  • Demonstrated ability to work in a flexible and innovative way in responding to consumer need.
  • Demonstrated experience in all aspects of case management including comprehensive assessment, planning, provision and evaluation of services for older people, people with dementia and or mental health concerns and their carers and families.

Successful candidates will be required to undertake probity checks including a National Criminal History Record Check.
