About the role

The purpose of the Child and Family Practitioner position is to provide a high level of skilled support and supervision to the placement of children and young people in a family-based care arrangement.  The successful candidate will hold a caseload of children and young people and will be required to build a rapport with the child/young person, conduct regular home visits, work with the care team and implement Looking After Children (LAC) requirements.

This position is being offered as permanent full-time, and can be based in either Sunshine and/or South Morang. This role will require travel across the metro area and as required to regional areas. There may also be a requirement to work outside business hours. A Tool of Trade vehicle (including fuel card) will also be allocated to the successful candidate.

Key Responsibilities

  • Develop and maintain functional and supportive relationships with children/young people, foster carers, families, and wider care team members in order to promote positive outcomes.
  • Working to KPI’s with a heavy focus on administrative reporting
  • Develop and implement care plans for children and young people in care, including monitoring their health, education, and behaviour needs.  
  • Provide ongoing support and supervision to carers
  • Support and monitor placements ensuring carers participate in case planning process
  • Participate in on-call roster as required

Skills & Experience

  • Degree in Social Work, Social Science, Psychology or Human Services
  • Experience within casework practice for children and young people
  • Experience working with children and young people in out of home care and Foster carers 
  • Experience developing quality case plans or individual plans 
  • Demonstrated experience working autonomously with multiple clients simultaneously
  • Experience working with individuals, families, or groups from a culturally appropriate perspective
  • High-level communication, influencing, liaison, negotiation, and mediation skills
  • Willingness to engage in LWB evidence-based trauma training.
  • Knowledge of the legislative requirements of supporting children and young people in out-of-home care.
  • Ability to maintain positive relationships with individuals, whilst being able to have honest and at times difficult conversations.
  • Current unrestricted driver licence

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
