About the role 

As a Case Manager within our Intensive Therapeutic Care team, you will play a play a key role in the lives of up to 6 Children & Young people residing in Orange. You will work closely with the Children, Young People and their support network to develop and implement therapeutic case plans that meet their individual needs and goals.

To be successful in this role, you will have experience working with Children & Young People in out of home care, previous experience managing complex caseloads, an understanding of trauma and its effects on children and young people. Regular travel to the locations of the Children & Young People is a requirement of this role.

Key Responsibilities  

  • Develop and implement high quality therapeutic case plans for Children and Young People in LWB's care 
  • Develop and maintain functional and supportive relationships with children and young people in therapeutic care as well as their families to support positive outcomes
  • Attend shifts within the intensive therapeutic care homes, and work closely with the team 
  • Meet with clients and family members to gather input for case plans and ensure their views are represented 
  • Coordinate and network with other agencies to effectively achieve case plan goals within tight time frames.
  • Participate in an on-call roster as required 
  • Ensure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service delivery 

Skills & Experience 

  • Relevant bachelor’s degree (Candidates with a relevant diploma, working towards a degree may be considered)
  • Previous case management experience within an Out of Home Care or Intensive Therapeutic Care setting
  • Understanding of current casework practices for children and young people 
  • Understanding of trauma and its effects on children and young people
  • Current Australian drivers’ licence
