About the role 

To be successful in this 12 Month Full Time contract, you must have previous experience as a case manager for young people in out of home care and a strong understanding of disability and the NDIS and funding packages. You will develop and implement therapeutic case plans for a case load of up to 6 young people within our Intensive Therapeutic Care, Significant Disability program in Wollongong, with occasional travel to Nowra.

Key Responsibilities

  • Develop and implement high quality therapeutic case plans
  • Record, monitor and review goals to ensure ongoing relevance to the young person
  • Attend shifts within the intensive therapeutic care homes
  • Meet with young people and family members to gather input for case plans
  • Coordinate and network with other stakeholders to achieve case plan goals
  • Participate in an on-call roster as required 

Skills & Experience

  • Relevant bachelor’s degree
  • Case management experience within out of home care
  • Knowledge of NDIS and funding packages
  • Understanding of current casework practices for children and young people 
  • Understanding of trauma and its effects on children and young people
  • Current NSW unrestricted drivers’ licence
