About the role

The Lifestyle Supports Coordinator is responsible for running Community Participation Programs that are centred around ensuing clients receive lifestyle support that reflect their NDIS Plan.  The role is responsible for organising and directing lifestyle support by instructing and coaching support workers. The incumbent demonstrates client centred, quality practice by engaging directly with clients, stakeholders and staff and they support client choice, and client and worker safety.

Key Responsibilities

  • Ensure that direct support is delivered, as far as possible, by the support worker
  • Running Community Participation Program
  • Complete service satisfaction activities as directed by the manager
  • Work with relevant staff to ensure that support is delivered to clients as per agreed NDIS Plan
  • Monitor and adjust support where required based on the NDIA plan, service agreement and client feedback
  • Manage rosters in line with NDIS funding
  • Be part of the on-call roster

Skills & Experience

  • Cert IV or equivalent in Disability, Mental Health or Community and Social Services
  • First Aid Certificate
  • Current Driver’s Licence
  • An excellent understanding of the principles of the BDIS and relevant legislation
  • Demonstrated coordination and leadership experience
  • Demonstrated knowledge of contemporary disability practice approaches

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
