About the Role

Key Responsibilities

  • Support the customer to implement their NDIS plan, including assistance to identify and select providers
  • Monitoring plan outcomes & plan expenditure to ensure the plan remains in budget
  • Strengthen and enhance the customer’s abilities to self-direct their own supports and participate in the community
  • Ensure mainstream services meet their obligations (i.e. housing, education, justice, health)
  • Ensure clear, accurate and timely record keeping including making information accessible to the customer
  • Capture any required data for the organisation and the Agency
  • Provide the NDIA , and the customer on request, reports on outcomes and success indicators within the agreed reporting frequency

Skills & Experience

  • Demonstrated experience in working with people with a disability, including supporting people with disability to develop positive relationships with a broad range of internal and external stakeholders
  • Relevant Tertiary qualifications including - Diploma in Disability; or o Certificate III or IV in Disability or equivalent; or o Certificate III or IV in Mental Health Qualifications

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
