About the Role

We are looking for a professional and positive individual to join our team as a Support Coordinator. In this role, you will assist NDIS participants in the support coordination and implementation of their funded NDIS plan to self-direct their own supports and select service providers. We are looking for someone committed to supporting and promoting the individual needs, skills, abilities, and personal goals of people with disabilities. Your prior experience supporting children, young people, and adults with disabilities is essential as well as comprehensive knowledge of the NDIS.

Key Responsibilities

  • Coordinate the supports for NDIS participants and ensure the services are within the budget
  • Build and maintain positive relationships with peers, clients, and external stakeholders
  • Report outcomes and success indicators to the NDIA within the agreed reporting timeframe
  • Travel to support our clients in your tool of trade vehicle to meet client expectations

Skills & Experience

  • Demonstrated experience supporting people with disabilities achieve their goals
  • An understanding of the National Disability Insurance Scheme objectives and principles
  • Excellent communication skills and the ability to engage with clients and their families
  • Strong written and computer skills for the creation of plans, programs, and reports

Successful candidates will be required to clear all state-based probity checks; including but not limited to a National Criminal History Record Check and NDIS Worker Screening Check.
