About the Role

In this role as Team Leader within our Aged Care team, you be responsible for overseeing the quality delivery of Commonwealth Home Support Programme (CHSP) services to clients throughout the Canberra region. You will work in partnership  with our clients and their representatives to develop and implement care plans which are meaningful, that reflects their individual goals and supports them to achieve positive outcomes. You will also take responsibility in leading and motivating a cohort of 20-30 community based support workers to deliver quality aged care support services.

Ideally, We are looking for someone with prior leadership/coordination experience within the Aged Care industry and a strong understanding of the Aged Care Standards and the Commonwealth Home Support Programme (CHSP).

This  position is offered on a Full-Time, Maximum Term Contract (to July 2022) with the ordinary hours of work are Monday - Friday during business hours. Please note that some work outside of core hours may be required to support the immediate needs of needs of clients and/or support staff.

Key Responsibilities

  • Coordinate and monitor high quality care and support services
  • Monitor CHSP individual costs, ensuring services are within budget constraints, guidelines, and Home Care Quality Standards.
  • Monthly data entry for reporting requirements.
  • Develop, lead, motivate and manage a small cohort of community-based support workers
  • Liaise with internal and external stakeholders.
  • Model appropriate conflict resolution, communication, and positive behaviour management skills, point of escalation and adjudication where relevant for client and direct staff feedback
  • Communicate and act in ways that are consistent with Life Without Barriers values: Responsive, Imaginative, Courageous, Respectful and Relationships

Skills & Experience

  • An unrestricted Australian Driver’s Licence
  • Demonstrated experience of working in a leadership or coordination role within the Care Industry
  • Certificate IV Community Services, Health, Nursing or similar, or equivalent experience
  • Demonstrated knowledge of person-centred thinking, Aged Care Standards and an understanding of CHSP.
