The role of the Home and Community Support Lead will play a vital leadership role in supporting people with disabilities to access support as well as oversees and coordinate the inclusive home and community support service focusing on providing one on one care in the Whittlesea community. This role is required to lead a team of home and community workers to deliver supports in the home and in a social/community support context.

Your responsibilities will include:

  • Plan and coordinate 1 on 1 in home and community support service including recruitment and marketing to participants/and their families.
  • Support clients and their support network to transition into programs/services.
  • Provide constant supervision/ check in of participants, ensure a high quality of care is being provided.
  • Provide regular opportunities for collating feedback, reflection, and learning.
  • Manage incidents by using required systems to report and follow up with investigations as required. Report service reportable incidents and restraints to the NDIS Quality and Safeguards Commission.
  • Undertake scheduling and management of staff and participants.
  • Provide staff supervision and support through the provision of regular meetings, appraisals, and communication. Ensure all relevant documentation is maintained, and resources/ training provided.
  • Ensure all NDIS processes are fulfilled including quotes, service agreements, service bookings, billing etc.
  • Independently complete monthly reports, impact reporting and any scheduled/ unscheduled requests
  • Support development or maintenance of service policies and procedures for delivering high quality supports, services, and programs to people with disabilities and carers.
  • Positively promote existing and new programs as required eg attending expo’s, networking events, maintaining social media posts and updating Website
  • Support the Community Services leadership team to create partnerships with other like-minded service providers in community awareness forums as well as ensuring all MOUs are created and current.
  • Support the team to advocate through appropriate forums on relevant services issues, gaps, and development opportunities for people with a disability.

The ideal candidate will have demonstrated experience in program coordination and leadership within inclusive community programs, be a collaborative innovative leader and mentor, possess strong written and verbal communications skills and excellent presentation skills as well as enjoy leading, motivating and mentoring a team and have a comprehensive understanding of the NDIS legislation and rules, the NDIS and use of NDIS portal. Demonstrated confidence in dealing with customers of different ages, cultures, ability levels and with different physical, social and communication needs will also be essential as is a Diploma in Community Services or equivalent qualification.

NB: The successful candidate will need to undergo an Employee Working With Children Check, NDIS Worker Clearance Check, First Aid, CPR and have a current Drivers Licence.
