The Outer Region Rough Sleeper Program (ORRSP) – Supportive Housing will be delivered through a unique multi-disciplinary Housing & Health Team, including Launch Housing workers, Bolton Clarke Nurse, MCM Youth worker, Ngwala Aboriginal worker, Neami Mental Health Worker and Monash Health Clinician. The teams will work intensively with people referred through the Outer Region Rough Sleeper Assertive Outreach Team to get and keep their housing.
The team offers a holistic, assertive outreach support service to people who are homeless and rough sleepers. It provides intensive, flexible, responsive, client focussed support featuring a multidisciplinary team approach with the aim of assisting clients to successfully sustain housing, improve physical and mental health and develop connections within the local community.
The role will be supervised day to day by the Launch Housing ORRSP Coordinator, and will receive youth-focussed support and supervision from the MCM Detour Team Leader, based in Frankston.
