Location: Work across Merri Health clinical sites and outreach as needed.

Your new organisation:

The model of care involves a Wellness approach, working with clients aiming to maximise functional independence and their ability to participate and remain living their own homes in the community.

Your new role:

You will be working as part of an established and supportive multidisciplinary team to provide direct and comprehensive high-quality and evidence-based podiatry services to clients aged 65 and over (or over 50 for Aboriginal and Torres Strait Islander People). This role specifically supports clients using Home Care Package funding.

You will:

  • Provide high quality and evidence based podiatry assessment and intervention for clients aged 65 and over
  • Liaise with clients Home Care Package Case Managers to recommend treatment and action plans for your clients
  • Maintain accurate, up-to-date clinical records to a high standard
  • Record activity data daily to ensure accurate and current activity date
  • Contribute to accreditation and quality assurance requirements and undertake quality improvement activities within the Aged and Primary Care division
  • Work alongside acute and tertiary partners to manage the care of high risk clients

You will be responsible for:

  • General treatment, orthotic prescription, nail surgery and diabetic assessment and education in both clinic and community environments
  • Wound and ulcer management
  • Biomechanical assessment and management of musculoskeletal disorders of the lower limb
  • Prescription of orthotic interventions
  • Assessment and management of neurological and circulatory conditions of the lower limb
  • Ensure health education is a significant component of each client contact
  • Liaise with Team Leader and clinical support Podiatrist on any clinical issues within the Podiatry service
  • Participate in 1:1 and group clinical support Podiatry meetings
  • Ensure the Podiatry service policies and practices are up to date with professional clinical practice
  • Provision of high quality, evidence-based services for all clients consistent with accepted professional and organizational policies and procedures.

To succeed in this role, you will have:

  • Be an enthusiastic Podiatrist who is engaged with your clients and puts their needs and goals at the forefront of you treatment plans.
  • The ability to advocate for your client and communicate with their case manager
  • Extensive knowledge, and experience, of managing a broad range of acute and chronic podiatry conditions
  • Experience in the application of the principles of Chronic Disease Management; Person Centred Care; Goal Directed Care Planning and Self-management.
  • Demonstrated ability to work independently and as part of a wider multidisciplinary team and health professional
  • Full Victorian Driving License
