Your new role

As a Service Coordination Officer you will be responsible for:

  • Providing seamless and timely brokerage services to Home Care Packages clients
  • Working collaboratively to ensure that outcomes are achieved in line with client goal directed care plans
  • Liaise with internal and external stakeholders to achieve best outcome for our clients
  • Maintain electronic data management system files and all relevant client and brokerage service documentation.

  • Well developed time management and organisational skills and ability to meet deadlines
  • Excellent verbal and written communications skills
