Your new role

As a Support Worker (Social Support Program) you will be responsible for:

  • Supporting clients across all service modalities of group and/or individual support including centre based and community based activities, telehealth and in home support
  • Developing a comfortable, safe, welcoming, flexible and innovative service environment
  • Actively engaging and supporting clients to develop and implement their Care & Goal Plans
  • Implementing wellness & reablement based activities, responsive to individual choices, decisions and aspirations, in a flexible and responsive manner
  • Ensuring compliance with relevant legislation, funding guidelines, service standards and contractual obligations

What you’ll need to succeed

To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We’re looking for individuals with:

  • Formal qualifications in aged care and experience in CHSP and/or HCP services
  • Sound knowledge of wellness and reablement principles & practice and person centred care
  • Strong interpersonal and communication skills with the demonstrated ability to engage with a diverse client base
  • High level of self motivation, enthusiasm and versatility
  • Computer literacy and knowledge of client information management systems
