As a Darebin Assertive Outreach Case Manager, you'll engage with individuals facing homelessness and provide critical support to help them stabilise their lives. Through community outreach, you'll connect people to housing, health services, and other vital resources, making a meaningful impact on their path toward independence.

We're looking for a candidate with a qualification in community services, social work or a related field, along with experience in assertive outreach or homelessness support. You should be collaborative, culturally sensitive, and committed to social justice with strong advocacy and case management skills.
