About the role

The Housing And Support Package (HASP)/ Supported Independent Living (SIL) program is a 24/7 residential support service for people living independently with mental illness, HASP supports clients with individualised services provided in line with their NDIS plan and unique needs. The program aims to promote greater self-management of mental and general health, increase access to the community and develop individual’s capacity to live independently with a focus on supporting people to achieve their better life goals.

We have an opportunity for a motivated, resilient permanent, full time Community Mental Health Practitioner to join the HASP team working a 24/7 rotating roster in the Glenside area. You will support customers to achieve their recovery goals through one-to-one support, facilitating peer learning workshops and assistance with daily tasks in a residential environment.

Key Responsibilities

  • Provide support to clients using targeted and integrated case managed responses.
  • Support clients by delivering high quality service and facilitating positive educational and social experiences.
  • Operate collaboratively with a range of services and the community to ensure a co-ordinated and integrated response to the client’s recovery goals.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers and key stakeholders.
  • Relevant experience in facilitating group activities for clients experiencing mental ill health.
