About the role

Mind Australia has formed a consortium with Launch Housing and other specialist agencies across the North East and Bayside areas of Melbourne, to assist people experiencing homelessness to transition from temporary hotel accommodation into housing as part of the Victorian Government’s From Homelessness to a Home (H2H) program.

We are seeking a resilient, compassionate Community Mental Health Practitioner for a 9 month fixed term, full time opportunity to make a lasting impact on homelessness in the Southern or Bayside suburbs area. Working from home, you will support vulnerable people obtain affordable housing and promote their health and wellbeing to prevent a return to rough sleeping. As this position requires travel within the Melbourne metro area a fully maintained company car is provided.

Key responsibilities

  • Provide outreach support to clients using targeted and integrated case managed responses.
  • Deliver high quality services and facilitate positive educational and social experiences.
  • Work as part of a multidisciplinary team offering specialised psychosocial support.
  • Collaborate with a range of services and the community to ensure a co-ordinated and integrated response to the client’s recovery goals.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in outreach work with people with mental health issues, issues relating to homelessness, complex needs and interacting with families and carers.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers and key stakeholders.
  • Relevant experience in facilitating group activities for clients experiencing mental ill health.
