About the role

The Mind Centre for health and wellbeing offers a targeted range of supports that aims to maximise client recovery outcomes through education and learning, mentoring, education and vocational support, housing and linkage services, counselling, NDIS planning and assistance delivered through Mind Recovery College. The Centre’s outreach services provides a person centred approach to recovery which is tailored to meet individual needs as they transition from the Community Care Unit and Adult Step Up Step Down program.

We are seeking a compassionate, self-motivated Community Mental Health Practitioner for a permanent, full time opportunity to work alongside the Mind Centre team in Cairns. With work from home options, you will provide holistic recovery-focused outreach supports to clients in their home or the community delivered face-to-face, online and over-the-phone. As this position requires travel within the region a fully maintained company car is provided.

Key responsibilities

  • Provide outreach support to clients using targeted and integrated case managed responses.
  • Support clients by delivering high quality service and facilitating positive educational and social experiences.
  • Work as part of a multidisciplinary team offering specialised psychosocial support.
  • Collaborate with a range of services and the community to ensure a co-ordinated and integrated response to the client’s recovery goals.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Able to provide a record of Vaccination Preventable Diseases or able to obtain vaccinations against Measles, Mumps, Rubella, Chicken Pox, Whooping Cough, Hepatitis B or other diseases as required by our Partnership with QLD Health.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Previous experience working in outreach and ability to implement a recovery oriented framework.
