About the role

Mind operate Supported Independent Living (SIL) and Short-Term Accommodation services for people with severe and enduring psychosocial disability, these residences are staffed 24/7 and funded through residents NDIS packages. Our practice is guided by Mind’s model of recovery-oriented practice, the SIL model and our dual disability framework; services operate in collaboration with the local community with a focus on supporting people to achieve their life goals.

We are seeking an experienced, resilient Community Mental Health Practitioner in the Chadstone area to work on 12 month fixed term, part time 30.4 hours/ week opportunity with a possibility of permanency, predominately afternoons from 2.30pm until 10.30pm. You will support our complex needs customers to achieve their recovery goals through one-to-one support, facilitating peer learning workshops and assistance with daily tasks in a shared living arrangement.

Key Responsibilities

  • Provide support to clients using targeted and integrated case managed responses.
  • Support clients by delivering high quality service and facilitating positive educational and social experiences.
  • Operate collaboratively with a range of services and the community to ensure a co-ordinated and integrated response to the client’s recovery goals.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field.
  • Expertise in working with people with mental health issues, complex needs, and collaborating with families and carers.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers and key stakeholders.
  • Relevant experience in facilitating group activities for clients experiencing mental ill health.
