The Sub-Acute short stay Youth Step-Up and Step-Down (YSUSD) program is delivered through partnerships with QLD hospital networks. Young people between the ages of 16-21 years are able to access to Mind staff 24 hours a day whilst supporting connections with family and friends as well as learning strategies to live fulfilling lives within their community. YSUSD provide a schedule of group activities alongside individual 1:1 support based on strengths and goals identified in recovery plans.
We are seeking compassionate, resilient Community Mental Health Practitioners for casual opportunities to work within a caring team on a 24/7 rotating roster in the Caboolture area. You will deliver recovery oriented, person-centred services to young people, families and carers and assist with recovery from mental health challenges by supporting young people to develop life skills in a residential setting.
