Mind’s Community Services, Country Wellness Connections (CWC) program provides psychosocial support services and community connection to individuals experiencing mental ill health in country South Australia. CWC services operate in partnership with consumers, families and carers to provide a range of non-clinical community-based support to help individuals achieve their goals, connect to services and improve quality of life.
We are seeking a self-motivated, dynamic Community Mental Health Practitioner for a permanent, part time (23 hours per week) opportunity in the Riverland area to provide services in line with Mind’s My Better Life Model. Working from home, you will play an integral role supporting individuals to achieve their recovery goals through 1:1 support, enabling shared supports and group work facilitation to enhance recovery, personal growth and activities of daily living.
