About the role

Mind’s Fee for Service program operates within the local community focusing on providing recovery focused mental health services, wellbeing, and ensuring customers and their families have the support and services they need to achieve their goals which reflect our Model of Recovery.

We have two opportunities for motivated, dynamic permanent, full time Mental Health Practitioners based at either the Heidelberg or Abbotsford office or with the flexibility to work from home. You will be responsible for providing short-term focused interventions, psycho-education, support and mentoring to customers with vulnerability and complexity.

Key Responsibilities

  • Deliver quality services and facilitate positive educational and social experiences to support customers.
  • Provide case management, outreach and support coordination services consistent with the defined goals identified in NDIS plans.
  • Develop strength based assessments as well as daily living skills and capacity for self-care with customers.
  • Work with families and carers by providing information, advice and support relevant to health and social needs.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Demonstrated ability to implement a recovery oriented framework and build strong relationships with service providers, agencies and key stakeholders.
  • Proven understanding of the Community Service sector, NDIS, TAC and WorkCover.
  • Experience working within a Fee for Service model and managing KPI’s is desirable.
