About the role

Mind’s Youth Psychosocial Recovery Service (YPRS) is a psychosocial support model for young people aged 16-25 years who have a mental illness and other complexity. The service provides intensive case management to support young people pre and post entry to a Youth Residential Rehabilitation (YRR) service and as an alternative to YRR placement.

We are seeking a passionate, self-motivated Youth Outreach Recovery Support Coordinator for 10 month fixed term, full time opportunity in the Traralgon area. You will engage with young people through assertive outreach coordinating wrap around supports and working holistically with individuals within a recovery-oriented framework.

Key Responsibilities

  • Provide support to clients using targeted and integrated case managed responses.
  • Support clients by delivering high quality service and facilitating positive educational and social experiences.
  • Work as part of a multidisciplinary team offering specialised psychosocial support.
  • Operate collaboratively with a range of services and the community to ensure a co-ordinated and integrated response to the client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with young people with mental health issues, complex needs and interacting with families and carers.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers and key stakeholders.
  • Experience in assessing need and working collaboratively to plan goal focused recovery using evidence informed approaches and tools.
