Youth Worker - Case Manager

  • Full Time - Fixed Term (12 months)

A key worker (Child Protection Contracted Case Manager) will provide effective case management to ensure young people receive integrated and coordinated quality services as they transition towards independence.

Flexible Funding Packages/Targeted Care Packages (TCPs), provide flexibility to tailored supports that meet the individual needs of young people and support alternate living arrangements outside of residential care such as; family reunification, home-based care, or independent living.

Key functions would include (but not limited to):

  • Directly engage and work therapeutically with the young person, their family, carers and others to facilitate changes that support achievement of the young person's case plan needs, goals and aspirations.
  • Liaise closely with care team, child protection and other professionals to support the achievement of the case plan.
  • Be the primary contact for the individual, family and children and collaborate with them to deliver a targeted care package, with a holistic approach of overall needs, goals, strengths, aspirations to achieve improved outcomes.
  • Lead and coordinate effective care teams for individuals, families, carers and children.
  • Work with young person and their families to facilitate all elements of the targeted care package and care and transition planning.
  • Intensive outreach to assist in the above core functions and conduct ongoing risk assessments.
  • Meet administrative requirements including; case note recording, care and transition plans, proposals, quarterly reports, case plan reviews, on call updates.
  • Works outside normal business hours if required on a rotational basis to provide phone support to Young People (under the age of 18) with Targeted Care Packages via the MOIRA After hours Oncall Support Service.

Qualifications & Experience;

A Tertiary qualification in youth work, counselling, community services or other relevant discipline and/or proven experience in the delivery of services to young people experiencing complex behaviours
