The role

Molonglo is looking for an experienced, compassionate, and proactive Case Manager for a fixed term of 12 months to cover maternity leave, with the possibility of extension or permanency.  

Your Place Housing Support Service is a Specialist Homelessness Service providing support to individuals and families from a range of different situations who are experiencing homelessness or at risk of homelessness.  

Working from our Queanbeyan office, upstairs in the Corporate Suites at Riverside Plaza, no two days are the same. The work is interesting, challenging and rewarding especially when you realise what you have put into place changes the lives of the people you are serving.

A typical week

Provide client-focused case management to assist people who are experiencing social disadvantage using a strengths-based approach. Typically, your day includes:

  • crisis intervention
  • preparation, implementation, and evaluation of case plans
  • practical and emotional support
  • coordination of services
  • networking and liaising with other agencies and services
  • writing case notes and administrative duties
  • assisting clients’ into independence
  • assisting with documentation
  • advocating for the client, family, and their children
  • visiting clients in their homes or at the office within Covid safe guidelines
  • assisting the Coordinator with bi-monthly and annual reports
  • assisting with immediate to long term accommodation options
  • supporting your colleagues
  • and from time to time you may organise and facilitate a focus group

Experience

Desirable: 

  • A qualification in Community or Social Work or similar discipline; 
  • A minimum 1 years’ experience in case management
  • Ability to work with vulnerable people using a strengths-based approach.

Essential:

  • Drivers Licence
  • Completion of, or current, NSW Working with Children Check 
  • National Police Check
  • Covid-19 Vaccinations
  • Eligibility to live and work in NSW

Required Skills

  • Excellent verbal and written communication skills
  • High degree of drive, initiative, motivation and outcome orientation, with the capacity to deal with multiple and often conflicting priorities
  • Ability to successfully work within a small team 
  • Ability to build rapport with clients
  • Experience working in partnership with a range of internal and external stakeholders
  • Well-developed computer skills, including proficiency with MS Office programs particularly Word, Excel, Outlook and Teams and ability to adapt and learn internal IT systems 
  • An awareness, understanding, and sensitivity to homelessness issues and the ability to communicate with a diverse range of people from various cultures
  • Ability to work calmly under pressure
