The Community Rehabilitation (Home Based) program offers rehabilitation services to clients within the context of their own home and community. The case manager undertakes the comprehensive assessment, develops the care plan and works closely with the interdisciplinary rehabilitation team coordinating therapy and planning for discharge. As an integral member of the team the successful applicant will have the opportunity to be a creative thinker and deliver holistic care solutions for clients in their own homes.
This is a permanent part time position (48 hrs / fortnight).
The successful applicant will have a professional qualification in Nursing or an Allied Health discipline, have broad experience in rehabilitation settings and possess sound knowledge of community resources. High level interpersonal and problem solving skills will be required. Regular supervision is provided.

In accordance with recent Victorian Public Health directions all employees working in the Public Health system need to be vaccinated against COVID-19 and be required to provide evidence of their vaccination status. All candidates that are recommended for appointment will be required to provide evidence they meet the vaccination criteria.
This will be by providing a copy of your immunisation history statement, which can be accessed via My Gov, or evidence that you have a vaccination appointment before 1 November 2021. This evidence will need to be uploaded to the E Recruit system.
