Acknowledging the vital role they play in providing healthcare to our community, Monash Health proudly supports the WHO 2021 International Year of Health and Care Workers.
The Integrated Community Health Team (ICHT) is offering a fantastic opportunity for a full time Team Leader. We are seeking a dynamic, enthusiastic and compassionate individual looking to contribute to our leadership team.
Monash Health is Victoria’s largest public health service. We are proud to provide healthcare to one quarter of Melbourne’s population, across the entire life-span from newborn and children, to adults, the elderly, their families and carers. More than 18,000 employees work at over 40 care locations across south eastern Melbourne, including Monash Medical Centre, Monash Children’s Hospital, Moorabbin Hospital, Dandenong Hospital, Casey Hospital, Kingston Centre, Cranbourne Centre, and an extensive network of rehabilitation, aged care, community health and mental health facilities.
The ICHT is committed to working with communities, families and individuals who have the poorest health outcomes and to providing high quality, multidisciplinary programs which embrace client centred practice and the social model of health. This includes working with people at risk of homelessness. We offer a variety of Allied Health and Nursing Services to clients within our catchment.
This role would suit anyone with an allied health or nursing clinical background looking to broaden their scope of practice into management. You will be responsible for quality service outcomes, target achievement and efficient staff and financial performance. Responsibilities also include line supervision of other staff and involvement in staff performance appraisal processes. Reporting directly to the ICHT Manager, you would be an integral part of this multi-disciplinary team. Additionally, you will maintain a small clinical caseload.
You will help manage and lead the team, providing support, supervision and facilitating effective change processes. Travel across Monash Health sites is essential to all roles within ICHT.
