Multicap is seeking a passionate and energetic individual with experience working with customers or clients in a support capacity to join our NDIS Support Coordination team in Rockhampton.

You will be joining an experienced and knowledgeable team who have a passion for working with customers, coordinating their supports under the NDIS. You may or may not have experience in the disability sector, we can help you build and apply this knowledge – we are really looking for someone who has experience supporting customers in a case management/coordination setting. You will be joining a growing organisation, that has been providing essential services in the community for over 50 years.

What you will do in the role

As our Support Coordinator, you will work closely with customers to understand their NDIS plan, engage with appropriate service providers and allied health professionals, and monitor progress toward the achievement of their plan goals. An important function of Support Coordination is to assist customers to identify community or mainstream supports that can assist them to achieve their set plan goals. This will include:

  • Work with NDIS customers and their families to assist them to meet their goals and get the best out of their NDIS plan enabling strong levels of choice and control
  • Develop sound plan implementation practices for the duration of the NDIS plan.
  • Build and develop strong relationships with customers and their families, service providers and allied healthcare professionals
  • Provide plan review reports to the NDIA clearly articulating the customers’ needs throughout the life of the plan and beyond.
  • Deliver services to customers which are effective, both responsibly and transparently.
  • Assist customers to build their capacity around the understanding of their plan and implementation.
  • Work well as a part of internal and external team, communicating customer needs
  • Work with the organisation to follow processes with innovation to develop as required
  • Follow best practice within the organisation ensuring that we maintain integrity around legislation and NDIS guidelines
  • Meet individual performance KPI’s as set by the organisation

What you will bring to the role:

  • Experience in case management skills (for example working with families, individuals or children) that can translate to support coordination
  • Excellent communication, organisation, flexibility and empathy to engage with multiple clients throughout the day
  • Ability and passion to support customers with a disability
  • Understanding of, or ability to, quickly understand the NDIS and support coordination framework
  • Ability to thrive in a fast-paced environment with continuous improvement of practices to ensure we are providing the best service to our customers
  • Experience coordinating the provision of services to customers and their families through networking and negotiation with service providers, allied health professionals and various stakeholders
  • High-level interpersonal skills and experience supporting people in situations of high emotions.

Mandatory requirement

The successful applicant will be required to obtain a positive notice from Disability Services (Yellow Card), Working with Children Check (Blue card) and National Criminal History check. You must also have NDIS Worker Orientation certificate completed and please login to https://training.ndiscommission.gov.au/ if you haven't obtained the certificate already.
