Multicap is seeking a passionate and energetic individual with experience working with customers or clients in a support capacity to join our NDIS Support Coordination team in Rockhampton.
You will be joining an experienced and knowledgeable team who have a passion for working with customers, coordinating their supports under the NDIS. You may or may not have experience in the disability sector, we can help you build and apply this knowledge – we are really looking for someone who has experience supporting customers in a case management/coordination setting. If you are interested in developing a career in the disability sector, this opportunity will provide support and learning to help you grow.
You will be joining a growing organisation, that has been providing essential services in the community for over 50 years.
As our Support Coordinator, you will work closely with customers to understand their NDIS plan, engage with appropriate service providers and allied health professionals, and monitor progress toward the achievement of their plan goals. An important function of Support Coordination is to assist customers to identify community or mainstream supports that can assist them to achieve their set plan goals. This will include:
The successful applicant will be required to obtain a positive notice from Disability Services (Yellow Card), Working with Children Check (Blue card) and National Criminal History check. You must also have NDIS Worker Orientation certificate completed and please login to https://training.ndiscommission.gov.au/ if you haven't obtained the certificate already.
