My Place

My Place is a not-for-profit provider of individualised and flexible supports to people with disability and their families.

My Place supports over 400 people with disability to live in their own homes, or remain in their family home, and to become valued and contributing members of their community. My Place does not support any group home, day centre of other congregate care arrangements. We support consumers in Perth Metropolitan Area, Great Southern region and the South-West region.

We are currently seeking applications for a full time Disability Services Officer in the metropolitan area.

The Officer will be responsible for supporting My Place Consumers to engage and manage appropriate supports.

Must have:

  • Certified Trainer Crisis Management De-escalation Training
  • Extensive Positive Behaviour Support background

Main duties will be:

  • Providing education and community access to people with intellectual, physical, social and emotional disabilities.
  • Interviews and assesses disabled Client’s nature and extent of difficulties
  • Interprets disabled clients needs into service plans, and implements educational, training and support systems and programs.
  • Selects and refers clients to suitable agencies that can provide appropriate help.
  • Monitors, evaluates and reports to appropriate Managers.
  • Analyses supporting resources for clients participants, such as community need and resources for health, welfare, housing, employment, training and other facilities and services involved.
  • Works in accordance with the medication administration policy, work health and safety procedures and client right’s policies including privacy and confidentiality.
  • Prepares submissions of client funding and resource claims, and reports to government and agencies.
  • National Disability Insurance Agency (NDIA) Portal Management.
  • Provides services coordinators with Positive Behaviour Training
  • Develops and implements Positive Behaviour strategies for the clients when required.
  • Preparing office procedures and administration tasks as necessary

The successful candidates will be offered an excellent salary package, the opportunity to salary sacrifice, access to a meals/entertainment card, five weeks annual leave and other allowances. Whilst this work is primarily community based, the successful applicant will be required to work from a home office for which a subsidy will be paid.

You will need to Possess a current Drivers Licence. Co-ordinators will also be subject the National NDIS worker screening process post 1st December 2020
