About the Role

The Allied Health Worker (Social Worker) - NDIS Transition role will be based at the Neami Hurstville site to deliver the Continuity of Support program (CoS). The CoS program provides psychosocial support to people who have previously accessed support from Partners in Recovery (PIR), Personal Helpers and Mentors (PHaMs), Day to Day Living (D2DL) and National Psychosocial Support – Transition (NPS-T) and have been found ineligible for supports under the National Disability Insurance Scheme (NDIS).

The CoS program provides a person-centred, individualised approach to support, based on consumer needs and preferences, within a recovery-orientated framework, while focusing on the strengths, abilities and resources of consumers to build resilience and increase capabilities and wellbeing through social and environmental opportunities.

The role will focus on providing NDIS re-testing support to consumers in the CoS program assessed as requiring ongoing and higher levels of support.

Core duties include:

  • Review NDIS application status of all participants in the CoS program
  • Where application have not been submitted, meet with staff and discuss barriers to submitting applications
  • Work with staff to fast track ARF process for CoS participants
  • Work with staff to identify barriers to engagements for CoS participants that are not engaged
  • Provide focused reflective practice sessions with staff around topics related to NDIS testing

What will you bring to the role?

  • Demonstrated skills in the provision of high-quality assessments and interventions to resolve or ameliorate emotional distress of consumers
  • Demonstrated ability to effectively perform in an environment of change, to be creative when problem solving and work to timelines
  • Demonstrated understanding of the issues relevant to people requiring NDIS assistance living in the community (including those from a CALD or ATSI background) and a strong commitment to consumer rights and their participation in their community
  • Understanding of the key issues affecting clients living in the community who are experiencing poor mental health and are struggling to access services
  • High level interpersonal skills, including the ability to communicate sensitively and effectively with clients, their families, friends and carers
  • Understanding of trauma informed care
  • Be a team player

Mandatory requirements

  • Social Work qualification
  • Membership with the Australian Association of Social Workers (AASW) – mental health accreditation highly regarded
  • Current NSW Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) – disclosable outcomes considered
