The Clinical Care and Coordination role will provide care coordination for people living with severe mental illness living in the community. You will promote care plans, risk assessments and facilitate access to appropriate clinical and community services.

About the Role

  • Part Time (0.8FTE), Maximum term contract until 30 June 2022
  • Based in Port Lincoln
  • Level 2 HPSS or Level 2 Mental Health Nurse
  • Above award salary with generous salary packaging
  • Triage and assessment of mental health referral
  • Foster and maintain relationships with external stakeholders to ensure good communication flow and clear referral pathways
  • Participate in community engagement activities and promote the mental health and wellbeing of the community

What you will bring to the role?

As the Clinical Care Coordinator you will have a professional background and be registered in your field, along with demonstrated experience.

Our ideal candidate will have a deep understanding of the issues faced by individuals in the region including people who identify from a CALD or Aboriginal and Torres Strait Islander background.

Other essential skills and experience we are looking for include:

  • The ability to work in a team environment
  • Excellent written and verbal communication skills
  • The ability to form and maintain strong relationships with key stakeholders
  • Knowledge of local services particularly relevant to the designated program area
  • Be able to work with individuals from diverse backgrounds
  • Be an honest and transparent individual

Mandatory requirements

  • Relevant Clinical qualifications and registrations: Mental Health Nursing (Level 2) or Mental Health Social Worker
  • Working with Children checks and Vulnerable Person check (required before commencement of work)
  • Australian Working Rights
  • Drivers Licence
  • Police Check
  • Criminal record check required before commencement of employment
